Welcome to 777 Health Center's website. We're glad you chose our site. It is an honor to have you with us. Let me take this time to introduce who we are.

777 Health Center is a 92 bed long term care facility located in Barcelona. We've been a part of the local community for many years and we pride ourselves in service to the area. We provide our residents with the best services available in the area from local physicians, nurses, certified nursing assistants, dentists, dieticians, pharmacists, activities, and more.

Our mission is to continuously work toward meeting the needs of our residents. Activities are developed by our Activities Director. These may include (but are not limited to) musical events, trips, games, crafts, and exercise class.

In situations where a resident needs assistance for social and emotional needs a full-time social worker is readily available. All of our residents are treated with care, dignity, and respect. Services
  • Physician:  Residents are seen by their attending physician or physicians extender at least once every 30 days. The attending physician is responsible for the resident's plan of care. Cialis super active 20mg, Revolution, or just another new hard on pill?

  • Nursing:  The licensed nursing staff consists of registered nurses (RNs) and licensed practical nurses (LPNs). Licensed nurses are on duty 24 hours a day.

  • Certified Nursing Assistants:  The greatest amount of contact you will have with the staff members will be with the nursing assistants. They will help you with your daily needs.

  • Dental:  Dental Services are available at the facility. Dentists are available for consultation.

  • Dietary:  Services are supervised by a registered dietitian (RD). Meals are prepared by qualified personnel. Any diet prescribed by the physician can be prepared by our staff.

    MEAL TIMES:  
    Breakfast 7:30 a.m.
    Lunch 12:30 p.m.
    Supper 5:15 p.m.
    Snack 10:00 a.m., 2:00 p.m., 8:00 p.m.


  • Social Services:  Support for social and emotional needs is provided by a full-time social worker. Her normal business hours are from 8:00 a.m. - 4:30 p.m., Monday through Friday, and on an as needed basis. When the facility cannot adequately care for the resident's social/environmental needs, she makes a referral to an appropriate agency.

    A social services consultant makes quarterly visits to the center in order to facilitate the development and implementation of the social work program.


  • Activities:  A planned activities schedule is prepared by the Activity Director. This may include (but not limited to) musical events, trips, games, crafts, exercise class, and books.

  • Laundry:  Upon request of the resident or family, 777 HC will wash the resident's personal laundry. All laundry is done by commercial equipment. All items should have the resident's name printed on them. A laundry pen is available upon request at the nurse's station. Labels are available at no cost to the resident to be ironed into residents clothing. This does not include ironing or sewing. When the family does the laundry, it must be picked up regularly to enable the resident to have clean clothing and to maintain our odor-free environment.

  • Housekeeping:  The staff is available every day to make sure 777 HC is kept clean and odor free.

  • Maintenance:  Qualified maintenance personnel are on duty Monday thru Saturday to make any necessary repairs on our equipment. They are on call 24 hours a day for emergencies.

  • Diagnostic/Rehabilitative:  This includes lab, respiratory therapy, physical therapy, speech therapy, occupational therapy, and x-ray. These services are provided by Ty Cobb Healthcare Systems, Inc., and must be ordered by the resident's physician.

  • Pharmacy:  Medications are dispensed through the TCHS Long Term Care Pharmacy.

  • Resident's Financial Accounts:  At the request of the resident or family, 777 HC will keep a resident fund account. Request for cash should be made to the administrative assistant. Money can only be obtained Monday-Friday during office hours, 8:00 a.m. - 4:30 p.m.

    A quarterly statement of the account is sent or given to the resident or responsible party.


  • Resident's Rights:  All residents have basically the same rights given to them as they had prior to admission. Upon admission, residents and/or their responsible party will be asked to sign a copy of these rights acknowledging their understanding.

  • Visiting Hours:  777 HC has no official visiting hours. Families are encouraged to visit their relatives whenever possible during the day and early evenings. The doors are locked at 9:00 p.m. Please do not leave small children unattended.

  • Trips out of the Facility:  Residents are allowed to make trips out of the facility. The person taking residents out must sign a form, at the nurse's station, taking responsibility for the resident while out of the building. Medicaid only allows a certain amount of overnight visits per year. Check with the Director of Social Services for the number of days allowed.

  • Room Assignment:  Residents will be assigned to their rooms without regard to race, color, and national origin.

    The Administrator, Director of Nurses and the Director of Social Services must coordinate room transfers. This decision shall be based on the evaluation of the specific resident's needs. This evaluation shall consist of roommate compatibility, safety of residents and staff, and medical necessity. Both residents and responsible party will be notified of necessity of room change. When necessary a 15-day written notice will be sent to the resident and responsible party in regard to room charge.


  • Food:  All food brought into 777 HC must first be approved by the charge nurse. Any food kept in rooms must be in sealed containers and only in small amounts at a time left with the resident.

  • Smoking:  The resident may smoke if accustomed to doing so. Smoking is only allowed in the designated areas, not in the resident's room. Tobacco can be brought in for the residents by family. All tobacco and accessories (e.g. matches, lighters, etc.) must be left at the nurse station.

    Employees and visitors are not allowed to smoke inside the facility.


  • Mail:  Personal mail may be sent and received unopened each day. Outgoing mail may be left at the nurse's station or Administrative Secretary. Volunteers are available to write letters upon request.

  • Office Hours:  The business office is open 8:00 a.m. to 4:30 p.m, Monday-Friday.

  • Tips:  No employee or volunteer of 777 HC is permitted to accept tips or gratuities.

  • Telephones:  The telephone company will install phones in the resident's rooms, at the resident's or responsible party's expense. A public telephone adapted for hearing impaired is located at the nurses station as well as a cordless phone available to residents as needed.

  • Transportation:  The Director of Social Service will assist in arranging transportation to doctor's office and to the hospital for routine appointments. The expense of the transportation, by ambulance services, is the responsibility of the family (if not covered by Medicare or Medicaid). The family is responsible for taking their resident for any appointment by car. Non-emergency transportation may also be arranged.

  • Personal Items:  If room permits, we encourage families to bring familiar items from home to provide a homelike atmosphere.

  • Discharge:  Discharge plans are done on admission and quarterly. All rehabilitation efforts will be made toward the resident's returning home. When this is not feasible, 777 HC will make recommendations for coordinated efforts to ensure that each resident has a planned program of continuing care.

  • Care Plan:  A plan of care is implemented on admission. This plan is a working tool that provides a profile of the needs of the resident, identifies the role of each service in meeting these needs, and the supportive measures each service will use to complement each other in the accomplishment of the overall goal of care. The resident and/or family member is invited to participate in the care plan meeting along with representatives from the departments of: Nursing, Activities, Social Services, Dietary, Physical Therapy and other departments involved in the care of the resident. Care plan meetings are held on Thursdays each week. Your family will be notified of the date and time of your review by a postcard.

  • Payment Policies:  777 HC is certified for Medicare, Medicaid, and private pay residents. Some insurance policies cover long-term care but special requirements often accompany the policy. When in question, contact the administrative secretary. Payments are expected on or before the tenth of each month and should be made through the business office at 777 Health Center.
Additional Information

  • Change in your physical status or condition. When you are admitted to 777 HC, one responsible party is designated as the person the staff is to call in case of emergency or change in your condition. Your charge nurse will make every effort to notify your responsible party should any sudden change in your condition occur. However, in case of an emergency and we are unable to contact your responsible party, necessary action will be taken for immediate treatment to preserve the life and welfare of the resident. Your responsible party will be notified as soon as we can reach him.

  • Clothing and Personal Laundry:
    • Female: Six to seven outfits should be available for the women. Families are encouraged to bring jogging outfits (or similar outfits) for incontinent residents. Two or three pairs of washable shoes will be sufficient.

    • Male: Incontinent men will need about seven pairs of pajamas, five robes, and two or three pairs of houseshoes.

    • All residents are dressed in street clothes in the daytime. Please bring street clothes as well as sleep clothes.

    • All items should have the resident's name printed on them in indelible ink. A laundry pen is available upon request at the nurse's station. Labels are made upon request to the Social Services department.

  • Electrical Appliances: Family members are asked not to bring the following items into facility: electric heaters, fans, hot plates, blankets, heat lamps, electrical extension cords, and chairs that are worn. Televisions and radios are allowed -- however, must be inspected and approved by engineering department. Personal furniture may be brought as space permits.

  • Beauty shop: Hair care services are available by licensed beauticians. They are not employed by 777 HC, but work directly for the resident. Price lists of beautician services can be obtained at the administrative secretary's office. Our Beauty Shop is open each week.

  • Storage:There is limited storage space available in the facility. We recommend that all luggage be taken home once the resident is unpacked. Please be certain that all belongings are taken out of a resident's room when a resident is discharged.
